How do I access Bayside Care and Support services?
Remember, you can contact our team at any time during assessment process for aged care services. If you have a question or need help accessing aged care support, we're here to help.
Step 1: Assessment through My Aged Care
Established by the Australian Government, My Aged Care is a free service that helps older people and their carers to navigate the aged care system. To check whether you are eligible for Commonwealth funded supports, you can either go to the My Aged Care eligibility checker or call My Aged Care on 1800 200 422. Once initial eligibility is determined, the My Aged Care team will arrange a free home assessment.
Step 2: Receive your assessment results
At your home assessment, your Aged Care assessor will advise you of the outcome and send you a letter outlining the details for either the entry level Commonwealth Home Support Programme or the additional support available through a Home Care Package. You can advise your assessor that you wish to receive services from Bayside Council if that is your choice. Bayside Care and Support does not have a waiting list for services.
Step 3: Review costs
While the government subsidises a lot of the costs, there might be a contribution required based on your approved care type and personal circumstances.
Step 4: Select your Home Care Provider
You have the right to choose your service provider. Bayside Care and Support offers home care services to Bayside residents for entry level services as well as neighbouring suburbs for home care packages. Contact us on 1300 870 744 to learn more about what we offer.
Step 5: Commence your Home Care Services
If you chose us as your provider, we will notify My Aged Care that you are commencing services with Bayside Care and Support and our team will work with you to develop a care plan.
What is the difference between the Commonwealth Home Support Programme (CHSP) and Home Care Packages?
The Australian Government's free service, My Aged Care will check your initial eligibility and determine which program or package is most suitable for you. If you have any questions before, during or after your assessment, you can always contact us on 1300 870 744 for clarity.
Commonwealth Home Support Programme (CHSP)
The Commonwealth Home Support Programme (CHSP) supports people with entry level care needs. If you need support with a few tasks at home to maintain your independence, you might be eligible for CHSP.
Home Care Package
A Home Care Package is designed for people who have a need for more assistance. Supports and services are flexible and customised to your individual care needs and go beyond what the CHSP can provide. You may be eligible for a Home Care Package if you require multiple care and support services on an ongoing basis to help you live independently and safely at home.
